Every individual, especially those with a disability, need a good plan for their life. I see Transition Planning, not only as a plan, but as a promise- a promise to make sure that your child has a thoroughly thought-out and supported plan for what their first day out of public school will look like. The "real world" can be a scary place; I would love to help ease some of the worries you might have surrounding your adult child's transition out of public school and into their life.

Utilizing my training in Person Centered Planning facilitation, I would love to work alongside you to create a plan for your child centered around what really matters- their strengths, abilities, preferences, available resources, and support network!
